Transcript
Request
Please print the form, complete all relevant information, then mail or fax the signed copy to
the Registrar's Office at the address or number below. Forms must be signed and the transcript fee must be received before
transcripts can be processed (usually 2-3 business days).

Note: A $5 transcript
fee per transcript must be received by the registrar's
office before the transcript request can be processed (usually 2-3 days). This
fee can be paid by check or by credit card (Visa/Mastercard). An
additional $5 fee is required for faxed transcripts. Due
to privacy requirements, e-mailed transcript request forms
cannot be processed.
