Health Evaluation Forms
Urbana University students are responsible for updating their health information as changes occur.
If your medical condition, emergency contact, immunization information, insurance coverage, or current medications have significantly changed, see the nurse in Blackmer Hall or email changes to nparks@urbana.edu.
Additionally, all full-time students must submit a copy of their insurance card by August 31 of each year to Health Services to be exempt from purchasing Student Health Insurance.
