I understand that admission to the B.S. in Nursing degree program is a two step-process.

Step One: Admission as a B.S. degree-seeking student at Urbana University. Upon admission, you will receive an acceptance letter to Urbana University.

Step Two: Matriculation in the Nursing Major. Required materials include (1) verification of current, unrestricted Ohio registered (RN) license, (2) evidence of a satisfactory background check in compliance with the Ohio Revised Code, (3) health assessment by physician or advanced practice nurse, (4) validation of immunizations (including Hepatitis B) and TB screening, (5) current CPR certification, and (6) verification of individual professional liability insurance. These materials are due to the BSN Program Director within 30 days of application for admission or by the end of the second week of your first UU class whichever is earliest. Upon matriculation in the Nursing major, you will receive a letter from the BSN Program Director.

By submitting you are agreeing that the information in this application is correct. Any falsification is cause for immediate cancellation of this application and registration at Urbana University.

In addition to this application for admission, required admission materials include payment of a non-refundable admission application fee, ACT/SAT scores (if under the age of 23 and have no previous college level coursework completed), official high school transcript and official transcripts from all previously attended colleges and universities. Freshmen and transfer students are required to pay a $25 application fee. Readmits, visiting, non-degree, education certificate, and post-baccalaureate applicants are required to pay a $15 application fee.
