Enrollment and Housing Deposit Form
Submit housing and tuition deposits to secure a spot in class.
Campus Life Information Form
All students are required to complete this form. Housing assignments will be made on a first-come, first-served basis.
Student Health Evaluation Form
Please be aware that residence hall access will not be permitted until this form is completed and returned. Be sure to review information regarding Meningitis and Hepatitis B and the medical insurance letter regarding coverage for students attending Urbana University.
Students who have been admitted to Urbana University and wish to receive information about auxiliary services, or accommodations and/or assistance from the Office of Disability Services are invited to complete this form.
